Cleveland Clinic Abu Dhabi operates a Case Management System within the Clinical & Nursing Institute, which allows us to carefully coordinate and plan every patient’s post-hospital care.
Our skilled Case Managers combine the skills of nurses, social workers, and administrators to assess many aspects of a patient’s wellbeing and personal situation. This may include their overall health, emotional and mental health, as well as their social and economic position.
Our Case Managers are trained to work closely with the patient, their family, and their entire healthcare team to ensure they are supported throughout their hospital journey and to identify any potential issues.
Case managers work across all medical institutes at Cleveland Clinic Abu Dhabi to support our patients. All inpatients are reviewed upon admission to ensure criteria are met and are continually reviewed throughout their stay to ensure they are discharged to the most appropriate environment at the most suitable time.
